Mụn không phải do mặt bẩn.

Why do we get acne? In short: acne starts when a hair follicle gets blocked by oil and dead skin cells. Inside that plug, C. acnes bacteria, which already live on the skin, multiply, the body responds with inflammation, and a red, swollen spot appears. Hormones, genes, stress, cosmetics, friction or Saigon's heat and humidity don't "create" acne on their own, but they can push those links harder. Acne is not caused by a dirty face.

First-time clients at Harmonie often ask: "I wash my face really well, so why do I still get spots?" Cơ chếMechanisms

Tại sao lại bị mụn? 4 cơ chế gốcWhy do we get acne? The four root mechanisms

Every pore is the opening of a hair follicle attached to an oil gland. Normally, oil travels up the follicle to the surface, carrying shed cells with it. Acne begins when that flow gets blocked, through four linked mechanisms.

1. Tuyến bã tiết quá nhiều dầu1. Oil glands produce too much oil

Oil glands respond to androgen hormones. Androgens rise sharply at puberty, which is why acne often starts then. The face, chest and back are rich in oil glands, which is exactly where acne tends to appear. Extra oil supplies material for plugs and a good environment for bacteria.

2. Tế bào chết kết dính, bít miệng nang lông2. Sticky dead skin cells block the follicle

The cells lining the follicle are constantly renewed. In acne-prone skin, they don't shed neatly but stick together and mix with oil into a small plug that seals the follicle opening. This is the starting point of every spot, including ones you can't see yet.

C. acnes bacteria multiply in the blocked follicle

C. acnes (Cutibacterium acnes, formerly called P. acnes) lives naturally on almost everyone's skin, including people without acne. In a blocked follicle full of oil and short of air, it multiplies and produces substances that trigger inflammation. That is why acne isn't contagious and isn't a sign of dirty skin.

4. Phản ứng viêm4. Inflammation

The immune system reacts to the trapped oil and bacteria: the area around the follicle turns red, swells, hurts and may fill with pus. The deeper the inflammation, the higher the risk of marks and scars, so don't squeeze inflamed spots with your fingers.

Hình thànhHow a spot forms

How does a single spot form?

  1. Vi nhân mụn (microcomedone): nút bít rất nhỏ, chưa nhìn thấy, là "hạt giống" của mọi loại mụn.Microcomedone: a tiny plug you can't see yet, the "seed" of every type of spot.
Open or closed comedone: the plug grows. If the opening stays open, the plug oxidises and darkens into a blackhead (not dirt); covered by a thin layer of skin, it's a whitehead; deeper and only felt as a bump, it's a closed comedone. See Blackheads vs whiteheads.
  3. Mụn viêm, mụn mủ: vi khuẩn sinh sôi, viêm bùng lên thành nốt đỏ, sưng, có thể có mủ.Inflamed spots and pustules: bacteria multiply and inflammation flares into a red, swollen bump that may fill with pus.
  4. Nốt sâu, mụn nang: thành nang vỡ sâu dưới da, viêm lan thành cục cứng, đau, dễ để lại sẹo. Kiểu này cần bác sĩ da liễu.Nodules and cysts: the follicle wall ruptures deep under the skin and inflammation spreads into a hard, painful lump that easily scars. This type needs a dermatologist.

The spot you see today usually started quite a while earlier, so any change to your routine needs weeks to months before you can judge it.

Yếu tố thúc đẩyContributing factors

Những yếu tố khiến mụn dễ nổi hơnWhat makes breakouts more likely

The factors below act on those four mechanisms, making skin oilier, follicles easier to block or inflammation worse. They usually add up.

Nội tiết và di truyềnHormones and genetics

Beyond puberty, many women break out in the days before their period. Adult acne, especially in women, often clusters around the chin and jawline, as What causes chin acne? explains. Your genes matter too: if your parents or siblings had severe acne, you are more likely to get it.

Căng thẳng, giấc ngủ và ăn uốngStress, sleep and diet

Ongoing stress and poor sleep can make acne worse for many people, and stress makes us touch and pick at spots more. On diet, the evidence is mixed: some studies have noted a link between sugary foods, refined carbohydrates or dairy and acne in some people, but individuals react differently. Observe yourself rather than going on an extreme diet.

Mỹ phẩm, sản phẩm tóc và ma sátCosmetics, hair products and friction

Kem dưỡng quá đặc so với loại da, kem nền không tẩy sạch, sáp hay dầu dưỡng tóc dính lên trán có thể góp phần bít nang lông; mụn lấm tấm dọc chân tóc thường gợi ý sản phẩm tóc. Ma sát lặp lại cũng vậy: khẩu trang, quai nón bảo hiểm, điện thoại áp vào má, chống cằm, quai balo cọ vào lưng.

Moisturisers too rich for your skin type, foundation that isn't fully removed, and hair wax or oils reaching your forehead can help block follicles; small bumps along the hairline often point to hair products. Repeated friction does the same: face masks, helmet straps, a phone pressed to your cheek, resting your chin on your hand, backpack straps rubbing your back.

Khí hậu nóng ẩm và mồ hôiHeat, humidity and sweat

Sweat mixed with oil and held against the skin under a mask, helmet or tight clothing makes clogging easier; many people find their acne worse in the hottest months. A gentle face wash or shower after heavy sweating helps.

Oral corticosteroids, some hormonal medicines, anti-epileptic drugs or lithium can trigger acne or make it worse in some people. Steroid creams, including "mixed" creams sold without a clear ingredient list, can also cause breakouts with long use on the face. Don't stop a medicine on your own; tell the prescribing doctor.

Hiểu lầmMyths

Những hiểu lầm hay gặp về nguyên nhân gây mụnCommon myths about what causes acne

"Mặt bẩn mới bị mụn""Only dirty faces get acne"

Acne forms inside the follicle, not from dirt on the surface. Washing twice a day and after heavy sweating is enough; washing more often or using harsh cleansers easily dries and irritates skin and worsens inflammation.

Hard rubbing and grainy scrubs damage the skin surface and can burst inflamed spots and spread inflammation, while the plug sits inside the follicle where scrubbing can't reach it. If you want to exfoliate, BHA at a moderate frequency is far gentler.

"Chấm kem đánh răng cho mụn mau khô""Toothpaste dries spots out"

Toothpaste is made for teeth, not skin; some of its ingredients can irritate, leaving the skin around a spot red, sore and more marked. With an inflamed spot, it's best to leave it alone and not squeeze it; a pimple patch keeps your fingers off.

Oil and water are two different things: oily skin can still be dehydrated, especially when you use acne actives, and skipping moisturiser easily weakens the skin barrier. Details in Why does oily skin still need moisturiser?

What a spa can help with, and when to see a dermatologist

A spa can extract blackheads, whiteheads, tiny comedones and ripe closed comedones using sterilised tools, offer peels to unclog pores or care for skin with mild inflamed acne, and advise you on home care. A spa does not diagnose, prescribe or deal with hormonal problems. For cystic, pustular or severely inflamed acne, Harmonie does not extract; please see a dermatologist first. You should also see a dermatologist if:

Nếu đang dùng thuốc bác sĩ kê, nhất là isotretinoin hay retinoid bôi, hãy báo kỹ thuật viên trước buổi làm để điều chỉnh hoặc hoãn lại khi cần.

If you are on medication from a doctor, especially isotretinoin or a topical retinoid, tell your therapist before the session so they can adjust or postpone if needed.

FAQFAQ

Câu hỏi thường gặpFrequently asked questions

Vì mụn hình thành bên trong nang lông, do dầu thừa, tế bào chết kết dính, vi khuẩn C. acnes và phản ứng viêm, không phải do bụi bẩn. Rửa mặt hai lần mỗi ngày là đủ; rửa nhiều hay chà mạnh chỉ làm da kích ứng thêm.

Because acne forms inside the follicle, from excess oil, sticky dead skin cells, C. acnes bacteria and inflammation, not from dirt. Washing twice a day is enough; washing more or scrubbing hard only irritates skin further.

The evidence is mixed. Some studies link sugary foods, refined carbohydrates or dairy to acne in some people, but not everyone. If you notice clear breakouts after a certain way of eating, try cutting back for a few weeks and reassess.

Adult acne is quite common, especially in women. Hormonal swings across the menstrual cycle, stress, pore-clogging cosmetics and friction from masks and helmets can all play a part. If acne comes with irregular periods, unusual hair growth or hair loss, see a doctor.

When you have many deep, hard, painful lumps, acne is leaving pitted scars, a few months of proper care haven't helped, acne comes with hormonal signs, or it appeared suddenly after a medication. A spa doesn't diagnose or prescribe.
